Med spa upsells: how I make each patient worth more without being pushy

Almost every clinic owner I talk to tells me the same thing.

"We can upsell anyone. We just need them in the door."

I believe them. The good ones really can. My job is getting women in the door. Their job is what happens next, and most of them are great at it.

But over six years and a lot of clinics, I have noticed what the very best ones do differently. None of it is pushy. Most of it is about timing.

Here is what I see.

The best upsell is not a sale

Picture this. A woman finishes her treatment. She sits up. Your injector hands her a mirror.

She looks at her own face. She smiles.

That moment is the most powerful moment in your whole clinic.

She is happy. She is looking at herself. She is thinking about her skin right now, not about her kids or her inbox. And she trusts the person standing next to her.

That is when the best clinics point out the next thing.

Not with a script. Not with a price list. Just something like, "You can see how much brighter this side is. When we do the pigmentation plan next, this spot here is what we will work on."

It does not feel like a sale. It feels like advice from someone who knows her skin.

Sell the package on the first visit

Some clinics wait. They do one treatment, see how she likes it, and maybe mention a package later.

I think that leaves a lot on the table.

The first visit is when she is most excited. She came in ready. She paid a deposit to be there. She has been thinking about this for months.

A package gives her a plan. Three sessions. Six sessions. A path to the result she actually wants, not a one time try.

What makes a package easy to say yes to:

  1. A clear result. Not "six sessions of microneedling." Instead, "smoother skin and smaller pores by spring."
  2. Real proof. Before and after photos from your own clinic. Reviews from women like her.
  3. A promise. What happens if she does not see the change? A guarantee that means something makes the decision easy.

Packages are also why I like running ads for package treatments. Body sculpting, microneedling, RF microneedling, and pigmentation packages are working really well right now. A woman who books one is already on a plan.

Pair treatments that belong together

Some treatments just go together. The clinics I see doing best have a short list of natural pairs and they mention them every time.

Here are a few common ones.

  1. Microneedling and a skin care routine to protect the result.
  2. Body sculpting for fat and a skin tightening treatment for the area after.
  3. A pigmentation plan and a daily sunscreen she will actually wear.
  4. Injectables and a skin quality treatment so the whole face looks fresh.

The pair should always make sense for her. If it would not help her, don't say it. Trust is worth more than one extra sale.

Book the next visit before she leaves

This is the simplest thing on the list, and it makes a huge difference.

Don't say, "Call us when you are ready."

Say, "Let's get your next one on the calendar. Does the same time in four weeks work?"

A woman who walks out with a date is far more likely to come back than a woman who walks out with good intentions. Life gets busy. The date on the calendar is what brings her back.

Make her feel like your patient, not the injector's

Here is one I see a lot. A patient falls in love with one injector. Then the injector leaves, and the patient goes with her.

The clinics that hold on to patients make sure she feels connected to the clinic, not just one person. A few ways I have seen this done well:

  1. Introduce her to a second team member on her first visit.
  2. Follow up from the clinic, not from one person's phone.
  3. Talk about "our results" and "our approach," not just one provider.

The math of a better patient

Here is why all of this matters, with made up numbers.

Say a woman books a $900 package. If she leaves with nothing else, she is worth $900.

Now say she adds a $150 skin care routine, books her next visit before she leaves, and comes back once more that year for another $900. She is now worth $1,950.

Same woman. Same ad cost to get her in. More than double the value.

That is why the clinics that are great at this can afford to spend more to get each woman in the door. And it is why I always ask what a patient is worth over a year, not just the first visit. More on that in Cost per booking: the only med spa ad number I watch.

It starts with who walks in

One last thing.

All of this works better when the right woman walks in. A woman who booked herself and paid a deposit is ready. She is not "just looking." She is already invested.

That is the woman who says yes to a package. That is the woman who rebooks.

That is why I focus so hard on getting the right women in the door. If you want to see how, start with The 300 Women Playbook.

Quick answers

What is the best way to upsell at a med spa?

Point out the next treatment at the moment she is looking at her own result. She is already happy and already thinking about her skin. It does not feel like a sale. It feels like advice.

Should med spas sell packages on the first visit?

Yes. When the offer is clear and backed by real proof, the first visit is the best time to sell a package. She came in ready, and a package gives her a plan instead of a one time fix.

How do I get med spa patients to come back?

Book the next visit before she leaves the room. A woman who walks out with a date on the calendar is far more likely to return than one who is told to call when she is ready.
