The 300 women

The women your referrals will never reach

By Mo, founder of IGX Marketing · October 22, 2026 · 5 minute read

A clinic owner told me something I hear a lot.

"Almost all of our patients come from referrals. They love us. They tell their friends."

She was proud of it. She should be. Referrals mean your work is good. Women trust you enough to send their friends.

Then she said the other part. "But some months it just dries up. And I don't know why."

Here is what I found when we looked closer.

Referrals are great, and they are small

Let me be clear first. Referrals are some of the best patients you can get.

She already trusts you before she walks in. Her friend told her you are great. She has seen the result on someone she knows.

Nothing is wrong with a clinic that grows on referrals. It just has a ceiling.

Here is the ceiling. A referral can only travel through a friend. So your referrals only reach the friends of your current patients.

That is one circle. A warm one. But a small one.

The women outside the circle

Now think about all the women near your clinic.

Some are friends with your patients. They might hear about you one day.

But most are not. They live a few streets away. They shop at the same stores. They scroll the same apps. They want the same treatments your patients get.

And they have never heard your name.

Not because anything is wrong with your clinic. Because nobody they know has told them yet.

I call these women part of your 300 women. They are the few hundred women nearby who would book this month if they knew about you. I explain how I find them in The 300 Women Playbook.

Why referrals come and go

This is why referral months go up and down.

A patient has a great result. She tells three friends. One books. Great month.

Then the next month, nobody happens to bring you up at dinner. Slow month.

You did not do anything different. Your work was just as good. The referrals just did not happen to flow that month.

I have seen this in a lot of clinics. The owner starts to wonder what she did wrong. She checks the reviews. She asks the team. Usually the answer is simple. Nothing changed inside the clinic. The friends who might have talked about her just did not happen to talk that month.

That is the hard part about referrals. You cannot turn them up when the calendar is slow. You just wait and hope.

What I add next to referrals

I never tell a clinic to stop getting referrals. Keep every single one.

What I add is a second way in. A steady way to reach the women who are not in that warm circle.

Here is how I do it.

First, I find the women nearby. I draw a circle around the clinic. Usually about 10 to 15 miles. I count the women inside it who book treatments like yours. I walk through that in How I count the women in a med spa's zip code in 20 minutes.

Second, I show them one clear offer. Not a menu. Not a discount. One real package for one treatment. Body sculpting, a microneedling series, or a plan for pigmentation. The women who want that thing stop scrolling.

Third, I ask for a deposit. She picks her time and pays a deposit to hold it. Facebook learns from what you ask it for. When you ask for deposits, it finds more women who pay.

The ad does what the friend used to do

Think about what a referral really gives a woman.

A friend tells her the clinic is good. She sees a real result. She feels safe trying it.

A good ad can do a lot of that too.

It can show real before and after photos from your clinic. It can explain what the package includes. It can show reviews from real patients. It can back the package with a real guarantee.

That is the same trust a friend gives. Just shown to a lot more women.

When she books with a deposit, she has already chosen you. She walks in a lot like a referral does. Ready.

If you want to see how I build an offer she will pay a deposit for, read How I get more women in the door at a med spa.

The best part: new women bring new referrals

Here is something I love about this.

Every new woman who comes in from an ad has her own friends. Her own circle. A circle your old referrals never reached.

If she loves her result, she tells them.

So the ads do not replace referrals. They feed them. Each new patient opens a new circle.

What happened with the owner

Once we added a steady way to reach new women, her slow months stopped being so slow.

She still got referrals. She still loved them. But now she was not waiting on them.

Results vary by clinic, by offer, and by market. But the pattern I see is the same. Referrals are a gift. They are just not a plan.

The short version

  1. Referrals are some of your best patients. Keep every one.
  2. But they only reach the friends of your current patients.
  3. Most women near you who would book are not in that circle.
  4. That is why referral months go up and down.
  5. Add a steady way in: one clear offer, shown to the women nearby.
  6. Ask for a deposit, so Facebook finds women who pay.
  7. Each new patient brings a new circle of friends.

Your referrals tell you the work is good. Now the women outside that circle just need to hear about you.

Quick answers

Why are referrals not enough to grow my med spa?

Referrals only travel through the friends of your current patients. That is a great group, but it is small. Most women near you who would book are not in that circle, so they never hear about you.

Should I stop relying on referrals for my med spa?

No. Keep every referral you can get. They are some of your best patients. Just do not make them your only source of new women. Add a steady way to reach women who do not know you yet.

How do I get new med spa patients who do not know anyone at my clinic?

Show one clear package offer to the women near you, and ask for a deposit to book. The women who are ready will stop and book. They come in having already chosen you, even with no friend to vouch for you.
